Cancer vaccine: Moderna and Merck announced overwhelmingly positive topline results from the Phase 3 INTerpath-001 trial, marking a historic moment for cancer treatment. Moderna stock, Nasdaq: MRNA, surged over 100 percent premarket.

The trial evaluated intismeran autogene, mRNA-4157/V940, an investigational messenger ribonucleic acid-based individualized neoantigen therapy, in combination with Merck’s blockbuster immunotherapy Keytruda – generic name: pembrolizumab – in patients with completely resected, high-risk stage IIB-IV melanoma.

The treatment actually showed a real, meaningful boost in survival rates, keeping patients cancer-free longer than Keytruda did on its own.

How the personalized cancer vaccine works

Intismeran is a made-to-order therapy, designed and produced based on the unique mutational "fingerprint" of a patient’s own tumor. Each treatment uses synthetic mRNA to code for up to 34 specific neoantigens – unique proteins on cancer cells caused by DNA mutations – training the patient’s immune system to recognize and attack residual cancer cells.

This is a huge shift away from the usual one-size-fits-all methods toward truly personalized cancer care.

The Phase 3 study enrolled 1,137 patients who were randomized 2:1 to receive the combination therapy or Keytruda alone following complete surgical resection

Implications and path forward

"This is the first Phase 3 study to show that intismeran … can reduce the risk of recurrence or death in patients with completely resected stage IIB-IV melanoma compared to KEYTRUDA alone," said principal investigator Professor Georgina Long.

The best part is that the combination didn’t show any new or scary side effects, staying right in line with what researchers expected. Now, the team is gearing up to share all the details at a big medical conference and start chatting with regulators about getting approval for the vaccine, hopefully through an accelerated review process.

This win shows a meaningful vote of confidence for Moderna’s other cancer projects and uncovers just how much mRNA could change the game.

A multi-billion dollar market opportunity beckons

Of course, on the other side of the table, on the profits one, analysts suggest the therapy could generate billions in annual sales.

Wall Street experts are already projecting blockbuster sales if intismeran secures regulatory approval. The personalized cancer vaccine market could be worth tens of billions annually, with melanoma representing just the first indication.

Moderna’s late-stage pipeline includes trials in non-small cell lung cancer, bladder cancer, and renal cell carcinoma; each representing massive commercial opportunities.

Moderna MRNA stock surged around 118 percent at its peak in premarket hours. Trading at $135 at the time of writing, keeping over a 115 percent jump.

For a company that was worth about $25 billion before this news, getting this platform to market across different types of tumors could totally rewrite its financial story and skyrocket what the company is actually worth.
