Moderna and Merck said on 19 August 2026 that their personalized mRNA cancer vaccine, now called intismeran autogene, met its main goal in a Phase 3 melanoma trial, cutting the chance that the disease returns after surgery when added to Merck’s Keytruda. Moderna’s stock more than doubled on the news, its best day ever, and headlines called it a landmark. That reaction is not wrong, but it is running ahead of the data. This is the first Phase 3 win for a class of medicine the industry has chased for a decade, and it matters. It is also an interim readout with no published survival numbers yet, in one cancer, for a therapy that still has to clear regulators and prove it works beyond skin.

What cancer is, and why beating it is so hard

Cancer is not one disease. It is what happens when a cell ignores the instructions that tell it when to grow, when to stop, and when to die, then copies that fault into its descendants until a tumour forms and, in the worst cases, spreads. Because the rogue cells are the body’s own, the immune system often treats them as friendly. That single fact is why cancer has been so hard to cure: the enemy looks like you.

Humans have known the disease for a very long time. The Edwin Smith Papyrus, an Egyptian medical text dating to roughly 1600 BC, describes tumours of the breast and notes there was no treatment. Around 400 BC the Greek physician Hippocrates named the swelling with its crab-like spreading veins karkinos, the root of the word carcinoma. For most of the centuries that followed, the only option was to cut the tumour out.

Real treatment progress is barely a hundred years old. Surgery became survivable with anaesthesia and antiseptics in the late 1800s. Radiation therapy followed the discovery of X-rays in 1895 and radium soon after. Chemotherapy arrived in the 1940s, when doctors noticed that mustard gas from the world wars destroyed white blood cells and turned the poison into a leukaemia drug. Then came targeted drugs like imatinib in 2001, and, most importantly for this story, immunotherapy: checkpoint inhibitors such as Keytruda, approved in 2014, which release the brakes that stop immune cells attacking tumours. Cancer vaccines are the next step, and they try to do something the earlier tools never did.

The disease is growing, not shrinking

For all that progress, the raw number of cancer cases keeps climbing. Global estimates put new cases at 5.9 million a year in 1975, 7.6 million by 1985 and about 8.1 million by 1990. The World Health Organization’s cancer agency then counted about 20 million new cases worldwide in 2022 and roughly 9.7 million deaths, and it projects around 35 million a year by 2050, a 77% rise in less than three decades. New cases have more than tripled in half a century.

Most of that increase is not because the world is suddenly sicker. It is because people are living longer and populations are ageing, and cancer is largely a disease of age. That is the uncomfortable backdrop to every vaccine headline: even a genuinely effective new therapy is arriving into a rising tide, and the burden falls hardest on lower-income countries with the least access to expensive, personalized medicine. A treatment tailored one patient at a time is a scientific triumph and, potentially, a distribution problem.

From a failing COVID story to a cancer bet

Moderna was founded in 2010 out of Flagship Pioneering, the venture firm run by Noubar Afeyan, on a then-speculative idea: that messenger RNA, the molecule that carries genetic instructions to a cell’s protein factories, could be used as a drug. The name is a contraction of modified RNA. Stephane Bancel took over as chief executive in 2011, and for years the company was long on promise and short on approved products. Its December 2018 listing was the largest biotech IPO in history, valuing a business that had never sold a single medicine at around 7.5 billion dollars.

Then COVID-19 changed everything. Moderna’s mRNA vaccine, developed at record speed, posted 94.1% efficacy and won emergency authorization in December 2020. Revenue exploded to roughly 18 billion dollars in 2021, and the stock peaked at around 450 dollars that year. But a vaccine for a receding pandemic is a shrinking business.
